About This Program
In collaboration with healthcare providers, Treasure Coast Food Bank’s FRESHCareRx program delivers life-sustaining support to neighbors living with chronic illness. Through medically tailored meals, personalized nutrition counseling, education, and wraparound services, FRESHCareRx helps improve health outcomes and quality of life.
Led by a dedicated team of nutrition and culinary professionals, program staff, administrators, and a Registered Dietitian, FRESHCareRx offers a continuum of care for homebound individuals facing illness.
Medically Tailored Meals are freshly prepared in Treasure Coast Food Bank’s production kitchen and delivered in bulk directly to participants’ doorsteps. Each meal is designed to align with treatment plans for nutrition-related conditions, ensuring consistent access to healthy food, essential for recovery and long-term wellness.
Partnerships:
Treasure Coast Food Bank is a proud member of the Food is Medicine Coalition (FIMC) and a recent graduate of the National Food is Medicine Coalition Accelerator Project, reinforcing our commitment to evidence-based nutrition interventions.
